We help seniors live well in the comfort of their own home.

The COVID-19 pandemic has led many of us to reflect on how and where senior care is delivered. Ohana Care is seeking to enable families to make the choices that best fit their circumstances. There are a lot of factors that need to be taken into consideration before making a final decision. Ohana can help you determine if home care services are right for you.

Personalized Care Plans Based on Your Unique Needs

Our care plans are designed to meet your needs.

1

Companionship Support

This plan includes companionship and covers any non-medical needs. Our companions utilize the Best Friends Approach and help with homemaking, transportation, leisure, meal preparation, pet care, companionship, and more. Dementia and Alzheimer’s care included.

2

Companionship and Personal Care

Our certified Health Care Aide (HCA) assists with additional care needs. For example, bathing, dressing, medication assistance, monitoring vitals, rehabilitation, etc. HCAs are typically capable of fulfilling both home care and home support plans. Dementia and Alzheimer’s care included.

3

Licensed Practical Nurse Care Plan

Licensed Practical Nurses (LPNs) provide additional medical care. These include hands-on head to toe assessments, in acute stages of medical concerns, some medication administration, and punctual needs. As well LPNs support community caregivers when required.

Registered Nurse Care Plan

For intensive care, a Registered Nurse (RNs) will attend to clients. RNs do home visits after major surgeries, during palliative care, to coordinate a care plan with health care professionals outside our organization, and to conduct assessments. Initial RN visit  is not billable.

Our Home Care Services

What our videos to learn more about home care and the types of services we offer

We provide your loved one with everything from basic to advanced foot care services, from assessments to treatment of many foot related issues. All of our services are provided by licensed professionals, and performed in the comfort of your own home!

These days, seniors can feel isolated. Our team specializes in companionship. Including taking walks, chatting, playing cards, going shopping. We will accompany your loved ones and make sure they are happy.

Our staff will make sure you are always dressed comfortably. Whether you are going shopping or just staying at home to rest. When we look good, we feel good.

It is important to have a helping hand every time our elderly loved one’s step in the shower; one slip could cause a serious injury. Our staff members are well trained and equipped in providing a comfortable bathing experience for your family member.

We will make sure you get the right medications on time, everytime. This is very important and we will make sure your loved ones are always keeping up with their medications so they can focus more on what they love doing.

Vacuuming, dusting and cleaning the bathrooms can be challenging to our elderly folks. We are committed to providing you a clean place everytime we go and see you.

When we get older, our bodies change the way we breakdown food. Our trained staff will help you prepare the kind of food you desire - once a day or the entire week - call us and we will setup a meeting to discuss your needs.

Need a break? Going away for a week holiday? We are here for you. Have us look after your loved one while you recharge. We will make sure they’re well taken care of.

Need someone to look after your loved ones around the clock? We send our staff to your residence to make sure you have a person looking after your loved one.

Just need to do a quick errand? We can help! Our staff will give you a hand in looking after your loved one. We require 3-hours minimum for each visit.

Our Health Care Aides also support the lifestyle of our clients by doing laundry, changing the beddings, respecting their regular routine, and being a companion using the Best Friends Approach™.

Health Care Aides are also used as companions when clients have “Unscheduled Care Needs”, meaning they will need support throughout the day that cannot be scheduled. These include incontinence, PRN medication (headaches, aches, constipation, diarrhea), transfers, asthma pumps, and more.

Licensed Practical Nurse (LPN)

Licensed Practical Nurses are more medically focused in their work with the clients. They may only spend one hour per visit simply to make sure there aren’t any drastic changes in the health of the client, ensure the HCAs are following the care plan, assist with more complex care needs, and assess the overall wellbeing of the client. LPNs may only visit a client right after they are back from the hospital, or when a new care plan is to be put in place.

During our first meeting, we will determine together what is best for you and your family. We will take into consideration your gender, medical conditions, and surrounding needs. Please let us know right away if you are not happy with the caregiver coming into your home. You should also feel safe, respected, and know you are in control.

Absolutely! We will also make sure to invoice you in the way that makes most sense to you to alleviate the stress of keeping track of all the invoices. We can print them, mail them, email them, and retroactively issue new ones.

We always charge for at least 3-hours, but we encourage you to use that time to its fullest potential. We can do a home safety check, clean the dishes, fold clothes, go over items in the fridge to ensure nothing is passed its shelf life, help the client make phone calls, assist with typing emails, and other odds and ends. However, some exceptions may apply to the minimum hours. Book your free assessment to find out more.

  • A binder can be left at the home of the client and state what care was provided and if there are any behavioral changes. This is the best option if you have multiple caregivers and family members.
  • Our care givers can call you from the phone at your loved one’s home and give you a run down before they leave (If you have Power of Attorney, or if a release of information form has been signed).
